Revision bariatric surgery is a second operation for patients who have already had a sleeve, bypass or gastric band. It is considered for insufficient weight loss, weight regain, reflux that medication does not control, stricture, or problems caused by an old band. It is technically harder than a first operation, takes longer and carries a higher complication rate. In Istanbul it takes 2–3 hours under general anaesthetic, with four nights in hospital.

Endoscopy, blood tests and, where needed, a contrast study or CT clarify the anatomy left by the first operation. Some patients are advised to follow a nutrition and behaviour programme instead of surgery.

Revision surgery
The 2–3 hour operation under general anaesthetic is planned around your existing anatomy and the problem being corrected. Adhesions can extend the time; a leak test is carried out during surgery.

You are monitored for four nights, then recover at your hotel. After a check-up and dietitian appointment you fly home at the end of the 7–9 days, with your surgeon's approval.
Aftercare & long-term tips
Your diet moves from clear fluids to purée, then soft and solid food, and this progression may be slower after a revision than after a first operation. Your supplements and blood test schedule are given to you in writing and followed up. Dietitian appointments continue for twelve months; call the 24/7 support line straight away if you develop a fever, abdominal pain or persistent vomiting.

Risks and an honest expectation
Revision surgery carries a higher risk than a first operation of leak, bleeding, stricture, clots in the leg or lung vessels, wound healing problems, permanent vitamin and mineral deficiency and the need for further surgery. For that reason the pre-operative assessment is more thorough, you stay four nights in hospital and a leak test is carried out during the operation.
