Risks and an honest expectation

An implant can fail to integrate with the bone, in which case it is removed and a second attempt made once the site has healed. Peri-implantitis, meaning inflammation and bone loss around the implant, is the main long-term risk and is driven by smoking and plaque. In the lower jaw the inferior alveolar nerve runs close to the site and injury can cause temporary or, rarely, lasting numbness of the lip and chin; in the upper back jaw the sinus floor can be perforated. A CT scan measures the distance to the nerve and the sinus before surgery, and the hygiene programme with annual review is part of the plan rather than an extra.
