Overview
DHI, or Direct Hair Implantation, loads each graft into the hollow tip of a Choi pen so that the channel is made and the graft placed in one movement. Because there is no separate channel-opening stage, grafts spend less time outside the body and the angle, direction and depth of each one is set individually. Placement is slower per graft, so a session usually moves 3,000–4,500 grafts: fewer than sapphire FUE, but with more control across the hairline and in thinning areas.

Blood tests and hairline drawing
On arrival day we run your bloods and draw the hairline. Whether the recipient area needs trimming is decided together at this appointment.

Extraction and pen placement
Grafts are extracted one by one with a micromotor punch and loaded into Choi pens. Opening the channel and placing the graft happen in a single movement at the planned angle.
- 04
First wash and flight home
The next day the first wash is done at the clinic, dressings come off and your aftercare instructions are handed over. You can travel on day three.
Aftercare & long-term tips
Follow the washing instructions to the letter for the first ten days; crusts fall away on their own, so do not scratch. If your recipient area was left unshaved, keep combs and hairdryer heat away from it for two weeks. Photo reviews are done remotely at months three, six and twelve.
First 24 hours
Stick to soft, lukewarm foods and drinks; avoid very hot or hard items while any numbness or sensitivity settles.
First few days
Ease back into normal eating as sensitivity fades — usually within a few days.

Risks and an honest expectation
The risks match sapphire FUE: temporary swelling, numbness, folliculitis and a proportion of grafts failing to take. What is specific to DHI is that the pen sets the depth, so placing too deep can cause buried grafts and cysts; depth is therefore calibrated for each patient.
