Examination and measurement
Nipple position and the volume to be removed are measured and the scars are drawn on so you can see where they will sit. Depending on your age and history, breast imaging may be requested before surgery.

The operation takes 3–4 hours under general anaesthetic. You spend the night in hospital, and a short-term drain is used where necessary.

Check-ups and flight clearance
Wounds and nipple circulation are reviewed regularly and bra use is explained. With your surgeon's approval you fly home on day 8–10.
Aftercare & long-term tips
Once home, wear the support bra day and night for the period advised and avoid upper body weight training and heavy lifting for six weeks. Keep the wounds dry; if a small opening appears where the vertical and horizontal scars meet, continue dressings and send a photograph. Keep up routine breast screening in your own country, and your histopathology result is sent to you.

Risks and an honest expectation
The commonest problem after breast reduction is wound breakdown where the vertical and horizontal incisions meet; this usually heals with dressings but can leave a thicker scar. Reduced or lost nipple sensation, reduced breastfeeding capacity, asymmetry between the breasts, fat necrosis producing firm lumps and the need to revise dog ears at the ends of the scar can all occur. Rarely the blood supply to the nipple is compromised, a risk raised by smoking, diabetes and very large reductions, which is why four smoke-free weeks and pre-operative tests are required.
